[1]- First, use file and new to open a new document size may be like 314x226 pixel with 96 resolution and RGBcolor mode, with white background
[2]- Now create a new shape help by the
Rectangular marquee
tool . As shown below

from the layer window
[4]- Now select the Linear Gradient tool
and choose the color
Black and Gray.
[5]- Fill the color help by the Gradien tool
after that press the Ctrl+D to deselect

a new shape with the Pen tool

press the "Delete" key on
your key board after that deselect

Gradient tool as shown below

with the Pen tool
[9]- Fill the color help
by the Gradient tool.
[10]- Now press the Ctrl+T and move it
help by the Curser as

after that
create a new layer from the layer palette
now fill the Black color deselect
[12]- Select the Blue color in the Forground color
from the
Tool bar and create a new line with the
Pen tool, as shown below after that
right click
and select the Stroke Path .

by the Elliptical marquee
tool and
create a new Layer3 from the
layer palette and now fill the Black
color
[14]- Your Layer3 should be
bitween layer2 and layer1.
[15]- Now click on icon
"Add vector mask".
[16]- Now go to Filter>Texture>Stained Glass
and use the
following settings
[17]- Now double click on this Layer3 and
select the Bevel and
Emboss from the
Layer style and use the following settings
[18]- Your image should look like this .
[19]- Double click on this layer1 and select the
Drop Shadow
from the Layer style now use
the following settings. After thate Repeat this
step to the Layer2 and Layer3.
